Under rule 14 of the Insurance Ombudsman Rules, 2017, you complain after you have written to the insurer, and you file within one year. The year runs from the insurer's reply, or from the end of one month after you wrote if it never replied (rule 14(3)(b)). You can file online at cioins.co.in, by email, by post or by hand (rule 14(2)). The ombudsman charges no fee.
The Rules list what a complaint must state and give no form. After registering a complaint, the ombudsman's office writes asking for Annexure VI-A, the form the Council for Insurance Ombudsmen publishes, with your documents and a signed copy of the policy within 10 days. The office closes the complaint if they do not arrive. The pack fills Annexure VI-A, with the form's own wording, from your facts.
The ombudsman also asks for your consent to mediate. A recommendation from mediation binds you only if you accept it in writing within 15 days (rule 16(2)). If you decline or mediation fails, the ombudsman passes an award. It cannot exceed the loss caused by the matter complained of, and never ₹50 lakh including expenses (rule 17(3)).
The pack also lists the documents to attach and gives your last date to file. The ombudsman cannot hear a matter that is before or decided by a court, consumer forum or arbitrator (rule 14(5)).

Common questions
Which ombudsman centre do I file with?
The centre whose area covers your home address or the insurer's branch you complain against (rule 14(1)). The Offices of Ombudsman page on cioins.co.in lists the 18 centres and their areas.
Do I have to write to the insurer first?
Yes. Rule 14(3)(a) needs a written representation to the insurer, and either its rejection, a reply that does not satisfy you, or a month with no reply. Online filing on the portal asks for a copy of that complaint.
Does the consent to mediation bind me?
The consent lets the ombudsman act as mediator, and its wording is unconditional and irrevocable, so sign it only if you want mediation. A recommendation binds you only if you accept it in writing within 15 days (rule 16(2)).
Can I take the matter to a consumer commission too?
Not while the ombudsman has it. Rule 14(5) stops the ombudsman from hearing a matter pending before or decided by a court, consumer forum or arbitrator, so choose one route.
Can I see the document before I pay?
Yes. Fill in your details and the page shows the first page of each document with your own answers in it. You pay only if it is what you need.
Can I change the document after I download it?
Yes. Each document comes as a Word file you can edit and a PDF you can print. You can also change your answers and prepare it again before you pay.
What if the payment goes through and the documents do not open?
The page checks the payment again and opens the documents on the same payment, so you are never charged twice. If they still do not arrive, write to us within 7 days and we refund the full amount.
Can you send or file it for me?
Yes. After you download, ask us to take the next step and we handle the sending or filing for you.
